Many mistakenly believe it to be a cataract complication, the return of their disease or the effects of a failed eye surgery, but in fact, it is an absolutely natural process. And the problem that has arisen can be solved very quickly, once and forever.<\/em><\/p>\n<p><strong><a href=\"https:\/\/okomedikas.com\/doctors\/pustovit-dmitro-yurijovich\/\">Dmitro Pustovit<\/a>, an ophthalmologist of Okomedikas Ophthalmology Clinic, answers our questions.<\/strong><\/p>\n<h2>What exactly is a secondary cataract?<\/h2>\n<p>&#8211; To begin with, I and my colleagues don`t like this term \u2013 \u201ca secondary cataract\u201d, because it distorts the very essence of the problem \u2013 it sounds as if a cataract grows back, a second time. In English the description is more accurate &#8211; \u201cafter-cataract\u201d, that is a condition that starts after cataract surgery.<\/p>\n<p>You must understand that it is neither a disease, nor a surgical complication (as one may think based on its name). This is a natural process of a body healing itself after surgery.<\/p>\n<p>What are secondary cataracts? As you know, the lens of an eye is located in a special capsule, which has the anterior and posterior walls. During lens replacement surgery the front area of the capsule is cut away so that the surgeon can remove the clouded natural eye lens through this little opening and replace it with an artificial intraocular lens. Over time, since the artificial lens is a foreign body and it comes into contact with the back wall of the capsule, some residual lens epithelial cells which remain after cataract surgery grow over the back (posterior) of the capsule to protect the living tissue from the foreign body.<\/p>\n<p>On the one hand, it is a natural protective reaction of the body. But on the other hand, these epithelial cells are opaque, which means that light rays are less able to travel through the eye to the retina. As a result, the eyesight is getting worse. This is what is called &#8220;a secondary cataract.&#8221;<\/p>\n<h2>How to treat secondary cataracts?<\/h2>\n<p>There are two treatment options. The old one is a surgery, during which the back of the lens capsule with a thickened layer of epithelial cells is dissected and removed. This procedure is technically complicated, it involves a rehabilitation period with significant limitations in daily-life activities. Besides, there is a risk of all kinds of complications such as inflammations, damage to the intraocular structures and intraocular lens dislocation.<\/p>\n<p>Therefore, a YAG laser capsulotomy is a more advanced and effective method of secondary cataract treatment. Basically, it goes as follows: a laser beam makes a little opening in the back wall of the lens capsule through which light normally enters the eye to get to the retina. This hole never closes. So, this procedure is done only once, there is no need to repeat it because in most cases visual acuity will be fully restored.<\/p>\n<p>It should be noted that the YAG laser is safe for the structures of the eye, has a precise focus point and only acts on specified areas.<\/p>\n<h2>What happens during surgery?<\/h2>\n<p>&#8211; At Okomedikas Clinic laser surgery for secondary cataracts is performed on an outpatient basis using anesthetic eye drops. The operation itself lasts no longer than 15 minutes, it is absolutely painless and our patient feels no discomfort, they only hear the characteristic laser clicking noise.<\/p>\n<p>The only &#8220;inconvenience&#8221; is that the procedure is carried out on a dilated pupil &#8211; so, first of all, eye drops should be instilled that will enlarge the pupil. These eye drops, as a rule, wear off by day`s end. That is why the maximum effect of the surgery may not manifest itself immediately, but the next day. Sometimes, there can be a side effect &#8211; eye floaters, which eventually disappear.<\/p>\n<p>There is no postoperative period either. The thing is that during surgery no incision is made that would open the eye`s interior to the external environment \u2013 that is why there is no risk of infection. But there can still be some risk of inflammation, so after the operation, the patient should put anti-inflammatory eye drops in 4 times a day for a week. Those are all the precautions after surgery.<\/p>\n<h2>Is secondary cataract inevitable after cataract surgery? And how fast does it develop?<\/h2>\n<p>&#8211; As for the inevitability of secondary cataracts, it is a matter of dispute. In the United States, for example, it is estimated that 100% of the people who underwent cataract surgery develop secondary cataracts. But in my experience, it is not always so. Yes, most people after cataract removal surgery develop to varying degrees some symptoms of secondary cataracts. But in some former patients they are more pronounced, in some less so, many don`t pay attention to them at all, and sometimes those signs do not prevent people from living the life they want \u2013 so no accurate statistics with regard to this problem are available.<\/p>\n<p>As for the timing of the process, it is very subjective and depends primarily on regenerative abilities of the body. So if a person is still young and their body&#8217;s ability to heal wounds is very high, secondary cataracts will develop very quickly. For example, in children (after congenital or diabetic cataracts) secondary cataracts develop inevitably and very quickly. And in general, the younger a person is, <strong>the more likely they will get a secondary cataract<\/strong> &#8211; because it is a protective reaction of the body, manifested by tissue regeneration.<\/p>\n<p>Moreover, the likelihood of developing secondary cataracts depends on the quality of intraocular lenses (artificial lenses). After implantation of Ukrainian lenses, secondary cataracts occur much more often, because they are rougher and less sensitive to laser exposure than imported high-quality lenses.<\/p>\n<p>But on average, <strong>a secondary cataract usually occurs 1 to 3 years later<\/strong>-the person begins to experience the same symptoms that led to the development of a primary cataract. That is, they feel like they are looking at the world through a dirty glass window or through fog.<\/p>\n<p>Since not many people know about the possibility of developing a secondary cataract, most people ignore the problem. And some of my patients simply didn`t know that they had a secondary cataract. In fact, it can be treated very quickly and effectively.<\/p>\n<h2>Why should people with this eye condition come to Okomedikas Clinic?<\/h2>\n<p>&#8211; First of all, so far we are the only clinic in Cherkasy Region equipped with the YAG laser to treat secondary cataracts; you will find it nowhere else.
